Appointment Information
How can I prepare for my appointment?
If you are visiting us for the first time, we ask that you arrive 10-15 minutes prior to your scheduled appointment time in order to fill out our client information form. You may also download it here and bring it with you. We prefer to have your pets' records faxed to us at least 24 hours in advance of your appointment so we can review the complete history in preparation for your visit. Please bring your pets' x-rays and any other images (ultrasound) with you to your appointment. We schedule all initial consultations for one hour, please be prepared to commit to this time with us.
If your pet is being referred for an ultrasound, anesthetic procedure, or if any labwork is expected, please fast your pet the night before (no food after 10pm, withhold water the morning of appointment. Do not fast if your pet is diabetic or prone to low blood sugar). We may also need to obtain a urine sample from your pet; if possible, please have your pet refrain from urinating just prior to the appointment. If diarrhea is a concern, a fresh fecal sample may be helpful.
What can I expect during my appointment?
A veterinary nurse will greet you and your pet and obtain vital signs, as well as a brief verbal history about your pet's medical problem. Although we may have acquired records from your referring veterinarian, it is important to hear about your pet's medical history in your words to give us a complete picture of the problem. The veterinary specialist will review the records, history, and any images prior to meeting you and your pet. A thorough physical examination will be performed and your pet’s condition will be discussed with you, along with any recommendations for further testing or procedures. A cost estimate is presented in all cases. If necessary your pet may be hospitalized for ongoing care or procedures. Your referring veterinarian will receive a phone update followed by a faxed letter summarizing your visit with us within 24-48 hours of your appointment.
Payment Information
Payment is due at the time of service. Peak Veterinary Specialists accepts cash, check, MasterCard and Visa. Payment plans are available through www.ChaseHealthAdvance.com and www.CareCredit.com, which offer financing options with low interest rates.
